Comment in: Optom Vis Sci. 2005 Jan;82(1):9; author reply 9-10. PURPOSE: To compare the agreement between subjective refraction and autorefraction using two commercially available autorefractors. METHODS: Prospective data were collected for 190 subjects using either the Nidek ARK-700A (Fremont, CA) or the Topcon KR-8000 (Paramus, NJ) and subjective refraction (masked to autorefraction). Refractions were compared in terms of spherical equivalent using Bland-Altman limits of agreement and astigmatic vector difference using median and 95th percentile. RESULTS: The two groups were similar for age, gender, spherical equivalent, and astigmatic power. The differences in spherical equivalent between subjective and autorefraction were significantly different (mean +/- SD; Nidek, -0.03 +/- 0.36 D; Topcon, +0.11 +/- 0.34 D; analysis of variance, F = 7.84; p < 0.01). However, the 95% limits of agreement were similar: Nidek, -0.74 to +0.68 D; Topcon, -0.55 to +0.77 D. The median differences in astigmatic vector difference were also similar: Nidek, 0.27 D and Topcon, 0.25 D. However, the 95th percentile was 0.67 D for Nidek and 1.09 D for Topcon. There was a low frequency of large (>1.00 D) differences in spherical equivalent, 1.1% with each autorefractor. There were five cases with astigmatic vector difference >1 D, all with the Topcon KR-8000 (5.3%). CONCLUSIONS: Both autorefractors show excellent agreement with subjective refraction. Despite a statistically significant difference in mean spherical equivalent (0.14 D), near identical limits of agreement (0.10 D difference) suggest clinical equivalence. Conversely, for astigmatism, despite similar median scores, major outliers were more likely with the Topcon, reflected in a 0.42 D larger 95th percentile, which suggests a small advantage for the Nidek for avoiding large astigmatic errors. Repeatability and validity of the PowerRefractor and the Nidek AR600-A in an adult population with healthy eyes. Allen PM, Radhakrishnan H, O'Leary DJ. Department of Optometry and Ophthalmic Dispensing, Anglia Polytechnic University, Cambridge, United Kingdom. p.m.allen@apu.ac.uk We assessed the repeatability and validity of the PowerRefractor and the Nidek AR-600A autorefractor. This is the first independent study conducted on adults to evaluate the performance of these instruments in a laboratory setting. Fifty subjects (23 males and 27 females) aged 16 to 61 years (mean, 37 +/- 12) participated in the study. The validity of the PowerRefractor and the Nidek autorefractor readings were determined by comparing them to subjective refraction. Measurements of refractive error were obtained from the two instruments on two separate occasions to assess their repeatability. The measured refractive error was converted into a dioptric power matrix for data analysis. No significant difference was found between the measurements obtained with the two instruments and the subjective refraction. The estimate of refractive error given by the two instruments was also found to be repeatable. In addition to measuring the refractive error, the PowerRefractor also offers the facility to measure eye position, pupil size, and dynamics of accommodation. The Vision in Preschoolers (VIP) Study Group conducted a multicenter, multidisciplinary, two-phase study to evaluate the performance of vision screening tests for identifying pre-school children with amblyopia, strabismus, significant refractive error, or unexplained reduced visual acuity (VA). The results of the VIP Study provide evidence-based guidelines for preschool vision screening. The best screening tests administered by eye care professionals were non-cycloplegic retinoscopy, Retinomax Autorefractor, SureSight Vision Screener, and linear, crowded Lea Symbols VA at 10 feet. The best screening tests administered by trained nurses and/or lay screeners were Retinomax, SureSight, and VIP single, crowded Lea Symbols VA screening test system at 5 feet. Eye care professionals can improve detection of strabismus by combining unilateral cover test with a refraction test and trained lay screeners can improve detection of strabismus by combining Stereo Smile II with SureSight. The best performing tests had high testability whether performed by trained eye care professionals, nurses, or lay screeners (>or=98%). Although very few children were unable to complete these tests, a child who was "unable" was much more likely to have a vision problem than a child who passed; therefore, children who are unable to complete one of these tests should be referred for further evaluation. When screening using the Retinomax, repeated testing to achieve the manufacturer's suggested confidence number is valuable and improves specificity. Federal initiatives to increase the number of pre-school children receiving vision screening or examination will increase the number of pre-school children identified with amblyopia, strabismus, and/or significant refractive error. Although there is general agreement regarding the importance of early detection of amblyopia, controversy exists regarding the importance of early detection of refractive error. Because of the high prevalence of significant refractive errors and lack of evidenced-based guidelines for correction of refractive error in pre-school children, future research is needed to evaluate the value of correcting refractive errors in preschoolers who do not have amblyopia and/or strabismus. Objective accommodation measurements in pseudophakic subjects using an autorefractor and an aberrometer. Win-Hall DM, Glasser A. PURPOSE: To compare objective measures of accommodation with the WR-5100K autorefractor and iTrace aberrometer to subjective push-down and defocus tests in normal phakic subjects and pseudophakic subjects with standard monofocal intraocular lenses (IOLs). SETTING: University of Houston, College of Optometry, Houston, Texas, USA. METHODS: The push-down test and defocus curves were used to stimulate and measure accommodation subjectively in pseudophakic subjects. For objective testing, a near target was pushed up and refraction measured objectively. For comparison, the same objective measurements were performed in phakic subjects. Calibration tests were performed with soft contact lenses in phakic subjects with varying refractive error and in pseudophakic subjects. RESULTS: Fifteen phakic subjects (mean age 28.9 years+/-5.52 [SD]) and 10 pseudophakic subjects (mean age 66.2+/-11.23 years) participated. Subjectively measured accommodative amplitude in the pseudophakic group was 3.28+/-1.11 diopters (D) in the right eye and 3.64+/-1.38 D in the left eye. Defocus curves in the pseudophakic group had a range of 2.00 D for distance-corrected visual acuity of 20/40 or better. Objective measurements in the pseudophakic group were 0.11+/-0.50 D with the autorefractor and 0.10+/-0.47 D with the aberrometer. Contact lens calibrations showed good agreement with the 1:1 line. CONCLUSIONS: The subjective tests overestimated accommodative amplitude relative to the objective measures. The autorefractor and aberrometer were accurate, reliable, and appropriate for objective accommodation measurements in pseudophakes. Plusoptix Vision Screener: the accuracy and repeatability of refractive measurements using a new autorefractor. BACKGROUND: The Plusoptix Vision Screener (PVS) is a new non-cycloplegic videoretinoscopy autorefractor. Refractive accuracy may affect its performance as a screening tool. AIMS: Study 1: To determine the intra- and interobserver variability of PVS measurements. Study 2: To compare PVS measurements with gold-standard manual cycloplegic retinoscopy (MCR). METHODS: Study 1: PVS refraction of 103 children with mean (SD) age 5.5 (0.6) years by two observers. Study 2: PVS and MCR refraction of 126 children with mean (SD) age 5.5 (1.5) years, including 43 children with manifest strabismus >/=5 PD, comparing mean spherical equivalent (MSE) and Jackson cross cylinders J(0) and J(45). RESULTS: Study 1: Repeatability coefficients (observer 1): MSE: 0.63 D, J(0): 0.24 D, J(45): 0.18 D; those of observer 2 were nearly identical. The mean difference (95% limits of agreement) between the two observers for MSE, J(0) and J(45) were, respectively, 0.03 (-0.62 to 0.68 D), -0.008 (-0.25 to 0.23 D) and 0.013 (-0.18 to 0.20) D. Study 2: MSE tended to be lower on PVS than MCR, with differences of up to 8.00 D. Less than 20% of values were within +/-0.50 D of each other. Agreement was better for J(0) and J(45). Strabismus was associated with an odds ratio of 3.7 (95% CI 1.3 to 10.5) of the PVS failing to obtain a reading. CONCLUSIONS: The PVS may underestimate children's refractive error.
